This Privacy and Cookie Policy explains how Summit Brain and Spine (“Summit”), a Utah corporation with headquarters in Lehi, Utah, collect, process, and use information about you when you use our website or any of our mobile applications, such as Summit Medical Institute (“SMI”).
Information Collected and Utilized
We may collect or receive information about you when you create your user account, check out as a guest and/or submit information, including, but not limited to, your first and last name, username, password, email address, street address, items purchased and/or stored in your virtual shopping cart, shipping preferences, and messages and emails you send to us. We may store additional information in your Summit Account for the purposes of verifying your identity and as required to use Summit’s applications.
Payment information is collected directly by a third-party processor through its secure link and only to facilitate the transaction. Specifically, no credit card information (including credit card numbers, expiration dates, billing, or other validating information) is transmitted, processed, or stored directly by Summit without fully and explicitly disclosing.
To the extent we retain personal profile information in your Summit Account, you can update your personal information by accessing the site and editing (where permitted) your personal profile.
We may use the information we collect or receive from you to sell you products and address your questions, and to manage our everyday business needs, such as to prevent fraud, resolve disputes, improve administration, create usage statistics, and comply with the law.
We may share customer information with other non-profit affiliated legal entities. In addition, we may provide your personal information to third parties that assist us in completing transactions, such as service providers that provide payment processing, maintenance, security, data analysis, hosting, and/or shipping services. These service providers are contractually required to adhere to industry standards in performing these functions and are prohibited from using your personal information for any purpose other than fulfilling our request for services.

Please be aware that any information you may voluntarily choose to display on any publicly available portion of SMI becomes publicly available and may be collected and used by others.
We may disclose your personal information or other content if required by a subpoena or other judicial or administrative order or otherwise required by law.
